A brief shot at the conclusion of Episode 15 depicts what has happened to Jemma Simmons in the reality of the Framework. She has died, which may make it difficult when Jemma tries to enter her avatar.

This quick shot of the headstones appears for only a few seconds at the end of the episode. It is presumed to have been filmed on the grounds at Culver Studios. There are certainly plenty of large patches of grass where they could setup several headstones to film such a quick shot.
